Clinical services which are provided through our center include care packages such as health promotion, comprehensive geriatric assessment including falls risk assessment, cognitive screening, nutritional assessment and frailty assessment. Center provides these services while clients are living in their own homes which would allow them to live an independent life with improved quality of life. This model of care would specially benefit older adults with social isolation and mild cognitive impairment.
Care packages include assessment by specialist physician, multidisciplinary assessment and refereals to others, family meetings, counselling and carer support.
Center for ageing research and ambulatory care welocome both local and international undergraduate students for short elective appointments and would facilitate elective projects.
We have launched this program on the 30th April 2025 aiming to provide essential knowledge and support to patients with autonomic dysfunction and their care givers.
